Don’t Delay โ€” These Conditions Are Treatable

Many men delay seeking help for scrotal and andrological conditions out of embarrassment or uncertainty about what to expect. This is understandable โ€” but it means many men suffer unnecessarily with problems that respond very well to treatment.

Whether you have noticed a scrotal lump, are experiencing persistent pain, have concerns about erectile function, or have been dealing with penile curvature, Mr. Jesuraj provides a thorough, expert assessment in a completely private and professional setting.

All andrological procedures at Tees Urology are performed by Mr. Manohar Jesuraj โ€” Consultant Urological Surgeon, FRCS Urol, FEBU โ€” not a junior trainee or GP with a special interest.

Seek assessment if you have:

  • A new lump, swelling or change in the scrotum
  • Persistent scrotal or testicular pain
  • Post-vasectomy pain that hasn’t resolved
  • Difficulty achieving or maintaining an erection
  • Penile curvature, pain or plaques (Peyronie’s disease)
  • Scrotal heaviness or dragging sensation
  • One testicle noticeably larger than the other
  • Concerns about fertility following vasectomy

Epididymal Cysts & Scrotal Lumps

Very Common

Epididymal cysts are fluid-filled sacs that develop on the epididymis โ€” the coiled tube behind the testicle. They are almost always benign but should always be assessed by a specialist to rule out other causes.

What you may notice

  • A smooth, round lump behind or above the testicle
  • Usually painless but can cause mild discomfort
  • May grow slowly over time
  • Often found incidentally

Management options

Scrotal ultrasound Watchful waiting Surgical excision
๐Ÿ’ง

Hydrocele

Highly Treatable

A hydrocele is a collection of fluid around the testicle inside the scrotum, causing painless swelling. It can occur at any age and is one of the most common scrotal conditions treated surgically.

What you may notice

  • Swelling of one or both sides of the scrotum
  • Usually painless โ€” may cause heaviness
  • Scrotum may feel like a water balloon
  • Can become large enough to cause discomfort

Treatment

Ultrasound confirmation Surgical repair Rapid surgical access

Varicocele

Affects Fertility

A varicocele is an enlargement of the veins within the scrotum โ€” similar to varicose veins in the leg. It is the most common correctable cause of male infertility and can also cause pain and testicular atrophy.

What you may notice

  • Dull ache or dragging sensation in the scrotum
  • Visible or palpable “bag of worms” above testicle
  • Discomfort that worsens when standing or exercising
  • Associated poor semen analysis results

Treatment

Doppler ultrasound Semen analysis Microsurgical repair

Erectile Dysfunction

Very Common โ€” Very Treatable

Erectile dysfunction (ED) affects men of all ages and is far more common than many realise. Rather than simply prescribing medication, Mr. Jesuraj investigates the root cause โ€” whether vascular, hormonal, neurological or psychological โ€” and tailors a treatment plan accordingly.

When to seek assessment

  • Difficulty achieving or maintaining erection
  • Reduced sexual desire
  • ED occurring in a younger man (under 50)
  • ED alongside cardiovascular risk factors
  • ED not responding to oral medication

Peyronie’s Disease

Specialist Assessment Essential

Peyronie’s disease causes the development of fibrous scar tissue (plaques) inside the penis, resulting in curved, painful erections. It is often poorly managed in primary care and requires specialist urological assessment and treatment.

What you may experience

  • Penile curvature during erection
  • Pain with erections โ€” especially in early disease
  • Palpable plaques or hard lumps in the penis
  • Difficulty with sexual intercourse
  • Penile shortening or indentation

Treatment options

Clinical assessment Oral therapy Injection therapy Surgical correction

Foreskin Problems โ€” Phimosis & BXO

Including Lichen Sclerosus

Phimosis is a tightening of the foreskin that makes it difficult or impossible to retract. Balanitis Xerotica Obliterans (BXO) โ€” also known as penile lichen sclerosus โ€” is a progressive scarring skin condition that can cause or worsen phimosis and, if left untreated, may affect the urethra. Both conditions are treatable but require specialist assessment.

What you may notice

  • Tight or non-retractable foreskin
  • Pain or discomfort during erection or intercourse
  • White, hardened or thickened skin on the foreskin or glans
  • Difficulty urinating or reduced urinary stream
  • Recurrent inflammation or balanitis
  • Splitting or cracking of foreskin skin
  • Urethral meatal narrowing (in advanced BXO)

Treatment options

Topical steroid therapy Preputioplasty โ˜… Circumcision Urethral assessment Meatotomy / meatoplasty Biopsy & histology

Important: BXO is a chronic condition requiring long-term follow-up. Circumcision is the most effective treatment for foreskin-limited disease, but if the urethra is involved, further specialist management is needed. Early diagnosis significantly improves outcomes.

Testicular & Post-Vasectomy Pain

Microsurgical Solutions

Chronic testicular pain (orchalgia) and post-vasectomy pain syndrome are poorly understood by many clinicians. Mr. Jesuraj has specialist microsurgical expertise in treating these conditions โ€” including microsurgical denervation of the spermatic cord where other treatments have failed.

What you may experience

  • Persistent dull ache in one or both testicles
  • Pain following vasectomy that hasn’t resolved
  • Pain radiating to the groin or lower abdomen
  • Discomfort worsened by activity or ejaculation

Treatment

Thorough investigation Medical management Vasectomy reversal Microsurgical denervation
