Epididymal Cysts & Scrotal Lumps
Very CommonEpididymal cysts are fluid-filled sacs that develop on the epididymis — the coiled tube behind the testicle. They are almost always benign but should always be assessed by a specialist to rule out other causes.
What you may notice
- A smooth, round lump behind or above the testicle
- Usually painless but can cause mild discomfort
- May grow slowly over time
- Often found incidentally

Hydrocele
Highly TreatableA hydrocele is a collection of fluid around the testicle inside the scrotum, causing painless swelling. It can occur at any age and is one of the most common scrotal conditions treated surgically.
What you may notice
- Swelling of one or both sides of the scrotum
- Usually painless — may cause heaviness
- Scrotum may feel like a water balloon
- Can become large enough to cause discomfort
Treatment
Varicocele
Affects FertilityA varicocele is an enlargement of the veins within the scrotum — similar to varicose veins in the leg. It is the most common correctable cause of male infertility and can also cause pain and testicular atrophy.
What you may notice
- Dull ache or dragging sensation in the scrotum
- Visible or palpable “bag of worms” above testicle
- Discomfort that worsens when standing or exercising
- Associated poor semen analysis results

Erectile Dysfunction
Very Common — Very TreatableErectile dysfunction (ED) affects men of all ages and is far more common than many realise. Peyronie’s Disease

Treatment options
Foreskin Problems — Phimosis & BXO
Including Lichen SclerosusPhimosis is a tightening of the foreskin that makes it difficult or impossible to retract. Balanitis Xerotica Obliterans (BXO) — also known as penile lichen sclerosus — is a progressive scarring skin condition that can cause or worsen phimosis and, if left untreated, may affect the urethra. Both conditions are treatable but require specialist assessment.
What you may notice
- Tight or non-retractable foreskin
- Pain or discomfort during erection or intercourse
- White, hardened or thickened skin on the foreskin or glans
- Difficulty urinating or reduced urinary stream
- Recurrent inflammation or balanitis
- Splitting or cracking of foreskin skin
- Urethral meatal narrowing (in advanced BXO)
Treatment options
Important: BXO is a chronic condition requiring long-term follow-up. Circumcision is the most effective treatment for foreskin-limited disease, but if the urethra is involved, further specialist management is needed. Early diagnosis significantly improves outcomes.
Testicular & Post-Vasectomy Pain
Microsurgical SolutionsChronic testicular pain (orchalgia) and post-vasectomy pain syndrome are poorly understood by many clinicians.
